Immunizations
Listed below are links to immunization forms that need to be completed before you begin your residency and/or fellowship training at The University of Arizona Health Sciences Center.
Please complete these forms, attach proof of immunizations and return them to the GME Office. Employee Health will review these forms and will provide any vaccinations that might be needed including a TB skin test. In addition, Employee Health will fit test the N95 Mask. THESE ARE MANDATORY REQUIREMENTS. TB TESTS ARE REQUIRED ANNUALLY.
